Chronic Disease Rehabilitation

A Chronic Condition

A chronic condition is a human health condition or disease that is persistent or otherwise long-lasting in its effects or a disease that comes with time. The term chronic is usually applied when the course of the disease lasts for more than three months.

Chronic diseases generally cannot be prevented by vaccines or cured by medication, nor do they just disappear.

Goals of Chronic Disease Rehabilitation

  • Learn to understand the condition and live successfully with it
  • Ensure safe activities of daily living within the limits imposed by any co-existing chronic disease.
  • Reversal and modification of known risk factors for cardiovascular disease
  • Improve quality of life
  • To improve functional capacity in respiratory patients and decrease cardiac events in diabetic patients
